S6 Graduates of the school year 2017-2018 should note the following：
- The Hong Kong Diploma of Secondary Education (HKDSE) Examination certificates are immediately available for you to collect in person from the Primary School Office until 31 August 2019. Unclaimed certificates will be returned to the Hong Kong Examinations and Assessment Authority (HKEAA) after 31 August 2019. If you have not collected the certificate, you are required to contact the HKEAA directly regarding the collection.
Collection Time: Monday to Friday (school days) 9:00 a.m. to 5:00 p.m. Saturday 9:00 a.m. to 12:00 noon (Except public holidays)
- You may arrange for an authorised person to collect the certificate on your behalf. Theauthorised person is required to produce your authorisation letter, aphotocopy of your identification document, and the original of his/her identification document for verification.
- You should collect the HKDSE examination certificate from the school as soon as possible to check the personal data shown on the certificate. Should amendments be necessary, please contact Ms. CHEN Wan Sze of the Primary School Office. Amendment of personal data on the certificate will be made by the HKEAA free of charge if the request is received on or before 31 December 2018. For amendment request made between 1 January 2019 and 31 March 2019, you will be charged a fee of $238 by the HKEAA. No amendment requests of personal data on the certificate will be accepted after 31 March 2019.
- If you lose your HKDSE certificate or wish to amend the personal data on the certificate after 31 March 2019, you may apply for a Statement of Results from the HKEAA. The current fee for the issue of a Statement of Results is HK$490 per copy.
